Created a masterlist of hypertensive individuals identified during the BP taking conducted per household

Conducted a weekly BP monitoring to keep track of blood pressure status of hypertensive individuals.

Conducted a mass BP taking during the general assembly on November 24, 2016.

Conducted a mass BP taking per purok on November 29-30, 2016.

Created a hypertensive spot map that will serve as a guide for monitoring purposes.

Establish a core group that will spearhead the projects and programs to be implemented.

Utilize the COMPACK program of DOH – sign up individuals with hypertension in the DOH Hypertension Club to avail free maintenance medicine.

"BP ating alamin, Hyprtension ating kontrolin" Outreach Program
January 20-21, 2018
the outreach program was conducted as part of the activities to help diagnose patients who is suspected of having hypertension. it aims to identify and properly diagnose hypertensive patients for them to be referred to RHU to have access to free medications.
Established Referral Scheme

After having the patient checked and diagnosed by a physician. The referral forms were given to the RHU. A referral scheme was established for easy access to medication. The MHO signed a memorandum of Understanding together withe the Medical students of ADZU for the dispensing of anti-hypertensive medications and anti-diabetic medications

The line graph shows a decreasing percentage of uncontrolled hypertension among the 114 identified individuals in the Barangay. As of September 2018, approximately there is a decrease case of uncontrolled hypertension from 80% to 70%.
